Claim handling
In case of an insured event, you should inform Nordea Life Assurance Finland Ltd “Lithuanian branch” within 30 days and indicate the insurer’s and insured party’s name and surname, personal code, and date and circumstances of the event. Information about the insured event may be provided as follows: at the nearest Nordea branch office in person, or by phone 1554, fax 8 5 236 27 86 or e-mail info.gyvybe@nordea.com.
You should bring the following documents to your nearest Nordea branch office:
- Documents verifying your identity,
- a completed application for receipt of the insurance benefit,
- the relevant extract from medical history about injuries, health examinations and treatment; and a document verifying the insured party’s disability (in case of insurance for disability or incapacity to work due to an accident),
- Death certificate and its certified copy (in case of death),
- Information about use of (or denial to use) the personal income tax bonus (in case of maturity of the insurance policy),
- Insurance policy,
- Other documents related to the insured event.
